Why does copy-paste fail when the PDF opens?

The visible page may be an image or may contain a damaged text layer whose character order does not match the page.

Should I run OCR again?

For a clean single-column scan, yes. Use deeper recovery when ordinary OCR loses structure, fields or reading order.

Does recovery guarantee exact text?

No. Outputs are prepared for human review and uncertain content remains flagged.
